Standing weekly appointments are recommended and every effort will be made to find a time that works for you. You may reserve your weekly appointment up to six months in advance to ensure that your time is available for you.

Fees

Regular Office Hours

Full Hourly Fee: $180.00
Some sliding scale available
Phone consultations (beyond routine scheduling) are billable
Cash, checks and PayPal accepted

Off-Hour and Emergency Appointments

Available at hourly fee of $200.00 per session
Off-hour appointments may be scheduled by calling (818) 497-2046

If you choose to pay online, please add $5.00 to your fee for processing.

Cancellations & Insurance

Cancellation Policy

All cancellations should be made by voice mail
Less than 24 hours notice:
You will be charged 100% of Regular Fee
Less than 48 Hours notice:
You will be charged 50% of Regular Fee

Insurance

Prior to setting your first appointment, please contact your insurance company for pre-authorization and to check your benefit level and copay
Please be prepared to pay your full rate at time of your appointment
A receipt will be provided to you for insurance reimbursement
